Change to our Opening times is coming!!

From Monday 16th September 2019 our opening times will change to 8am - 6.30pm. From this date our telephone lines and doors will be open for patients to access and book our services.
PLEASE NOTE 
For medical advice when the surgery is closed please call NHS 111 or logon to 111.nhs.uk If your call is an emergency please call 999.
Practice Closures  2019/2020- Protected Learning Times for Hough Green Health Park
Thursday 28th November closed from 1pm
Wednesday 29th January closed from 1pm
Thursday 27th February closed from 1pm
Wednesday 25th March closed from 1pm
Thursday 30th April closed from 1pm
On the above dates the practice will be closed from 1pm and will re-open the following day. If during these times you require urgent medical advice or attention please call 111.  Call to NHS 111 are free from both landlines and mobiles. You can also access NHS 111 online by typing 111.nhs.uk

Hough Green Health Park

Formerly Upton Medical Centre, Hough Green Health Park is proud to offer the highest standard of patient-centred healthcare.

We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

In addition to everything you need to know about the practice you will also find a wealth of health-related information in the menu on the right hand side.

FLU Vaccine

Aged 65 plus

Vaccines will be available for patients aged 65+ from 24th September.  Please call to book an appointment for one of our flu clinics, which are filling up fast.

Aged 18-64 at Risk

Vaccines will be available from 11th October for patients who are aged 18-64 and are recorded at risk, this will included patients who have Diabetes, COPD, currently Pregnant plus other conditions. If you are unsure if you quality for a free vaccine please call the practice who will advise if you are eligible or not.  If you not eligible you can have the vaccine at selected local pharmacies for a small charge.

Pregnant

Vaccine is available for patients who are currently pregnant, please call practice to make an appointment.

Carer

If you care for someone and are registered as a carer you are entitled to a Flu vaccine, please check with practice if you are unsure.

Child's Flu Nasal Spray

All children aged 2&3 will be invited to practice to receive the nasal spray.  Children who are age 6months - 17 years and at risk will also be invited to practice to receive the nasal spray.

PLEASE NOTE ALL OTHER PRIMARY SCHOOL AGED CHILDREN WILL BE GIVEN THE NASAL SPARY BY THEIR SCHOOL. PLEASE ASK YOUR SCHOOL WHEN THEY WILL BE GIVEN. PLEASE NOTE WE DO NOT HAVE THE VACCINE STOCK TO GIVE SCHOOL AGE CHILDREN THE NASAL FLU IN PRACTICE THIS MUST BE DONE AT SCHOOL.

Friends & Family Test (FFT)

The NHS Friends and Family Test (FFT) is an important opportunity for you to provide feedback on the care and treatment you receive from the NHS and to help improve services. From 1 December 2014, NHS England has made it a contractual requirement that all GP Practices undertake the FFT.
In order to provide this feedback, you can CLICK HERE which anonymously sends your response or you can pick up the questionnaire in the Practice and complete and post in the green posting box in the Practice Reception Area.

Onlne Access to Medical Records

We now offer Online access to your medical records at the Practice . This means that you are able to access the information you need, to make the right choices about the care you receive, giving you increased control over your own care and care records. Access will allow you to book appointments, order your repeat medication easily and quickly, over the internet from home, work or on the move, view some of the electronic medical information held about you in the GP system. You will be required to register for online access, and will be given a password to activate your account. Please go to www.patientaccess.com for more information.
