We now have an appointment reminder system set up at the Practice.  This will enable your appointments and reminders to be sent to you via text message.  Please update your mobile phone contact details with the Receptionist at your earliest convenience to take advantage of this great new service.

Practice Closures - Protected Learning Times for Hough Green Health Park
Wednesday 26th September closed from 1pm
Thursday 25th October closed from 1pm
Wednesday 28th November closed from 1pm
On the above dates the practice will be closed from 1pm and will re-open the following day. If during these times you require urgent medical advice or attention please call 111.  Call to NHS 111 are free from both landlines and mobiles. You can also access NHS 111 online by typing 111.nhs.uk

Hough Green Health Park

Formerly Upton Medical Centre, Hough Green Health Park is proud to offer the highest standard of patient-centred healthcare.

We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

FLU Season 2018/19

Vaccination for over 65’s

A more effective flu vaccine is available this winter for those aged 65 and over, which could prevent deaths and reduce the impact on the NHS.

You are eligible for the new flu vaccine this year if you will be aged 65 and over on March 31 2019, that is, you were born on or before March 31 1954. So, if you are currently 64 but will be 65 on March 31 2019, you do qualify.

Who else is eligible?

Those eligible for the flu vaccine this year include:

• adults 65 and over (including adults over 18 at risk of flu)

• pregnant women

• children aged 2 and 3

• children in reception class and school years 1, 2, 3, 4 and 5 (these will be done through your child's school)

• children aged 2 to 17 years at risk of flu

It is also available for children 6 months and above in an eligible group who cannot receive the live vaccine. If you have any queries regarding eligibility please contact our Reception.

Children’s Vaccination

A live quadrivalent vaccine (which protects against 4 strains of flu), can be given as a nasal spray. This is for children and young people aged 2 to 17 years eligible for the flu vaccine

If your child is aged between 6 months and 2 years old and they are in a high-risk group for flu, they will be offered an injected flu vaccine as the nasal spray is not licensed for children

 

PLEASE NOTE: ALL ELIGIBLE patients are able to attend our flu clinics, you will receive a text message or a letter if you are eligible, once you have received your letter or text please contact the surgery on 0151 5115805 to book the next available appointment.  If you are unsure please contact our reception team.

Friends & Family Test (FFT)

The NHS Friends and Family Test (FFT) is an important opportunity for you to provide feedback on the care and treatment you receive from the NHS and to help improve services. From 1 December 2014, NHS England has made it a contractual requirement that all GP Practices undertake the FFT.
In order to provide this feedback, you can CLICK HERE which anonymously sends your response or you can pick up the questionnaire in the Practice and complete and post in the green posting box in the Practice Reception Area.

Onlne Access to Medical Records

We now offer Online access to your medical records at the Practice . This means that you are able to access the information you need, to make the right choices about the care you receive, giving you increased control over your own care and care records. Access will allow you to book appointments, order your repeat medication easily and quickly, over the internet from home, work or on the move, view some of the electronic medical information held about you in the GP system. You will be required to register for online access, and will be given a password to activate your account. Please go to www.patient.emisaccess.co.uk for more information.
