To register with Hough Green Health Park you must be residing within our practice boundary.
As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the Practice Nurse or Health Care Assistant for a routine new patient health check.
We will require two forms of ID for new registrations and your NHS Number, please contact the reception team for details of acceptable forms to bring with your completed registration form.
Medical treatment is available from the date of your registration being linked to our clinical system. Not the date you submit you registration forms.
Patients who move out of our boundary area of postcode WA8 are asked to register with a new GP closer to their new address as soon as possible.
If you are a Carer you should register with the Halton Carers Centre. They are a great source of support and advice. Please ask at Reception for a form.
If you are serving, or have ever served, in the Armed Forces including the Territorial Army (TA) please notify the Reception staff. It will be entered onto your record. If you ever suffer from a condition that could be related to your service then you will receive priority treatment from the NHS.
