New Patient Registration

registration1The doctors welcome new patients who live within our practice boundary area of Widnes with postcode WA8.

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the Practice Nurse or Health Care Assistant for a routine health check. 

We will require two forms of ID for new registrations, please contact the reception team for details of acceptable forms to bring with your completed registration form.

Medical treatment is available from the date of registration. 

Patients who move out of our boundary area of postcode WA8 are asked to register with a new GP closer to their new address.

When you are registering, can we ask you to give some thought to joining the Organ Donation Register?  There is an acute shortage of organs which are desperately needed by critically ill patients throughout the UK.

If you are a Carer you should register with the Halton Carers Centre. They are a great source of support and advice.  Please ask at Reception for a form.

If you would like to have some input into the health services provided in this area (not just in the GP practice) please consider joining our Patient Participation Group, either in person or on-line.  

If you are serving, or have ever served, in the Armed Forces including the Territorial Army (TA) please notify the Reception staff.  It will be entered onto your record.  If you ever suffer from a condition that could be related to your service then you will receive priority treatment from the NHS.
